Please go to the Programs > Therapeutic Riding link on the website and download and print a Rider Application Form. We are currently accepting applications for the 2010 programs that usually start in May (start dates depend on ground conditions and class availability) and run through the fall.
Applications are available on our website at http://www.lifestriders.org/Riders.htm
Applications must be completed and mailed in to our office (no faxes please). Please call for detailed instructions on medical history forms needed.
